UNDERSTANDING YOUR OPTIONS
EKG vs Holter monitor — which test is right for you?
Both tests measure the electrical activity of your heart — but they serve different purposes. Your physician will determine which is appropriate based on your symptoms, history, and what needs to be evaluated.
|
⚡ ELECTROCARDIOGRAM (EKG / ECG) A snapshot of your heart’s electrical activityAn EKG records the electrical signals your heart produces over a period of about 10 seconds. Electrodes are placed on your chest, arms, and legs, and the machine produces a tracing of your heart’s rhythm and electrical conduction. The entire test takes about 5 minutes and results are reviewed immediately. Commonly ordered when: →
Chest pain, pressure, or tightness
→
Shortness of breath or fatigue
→
Palpitations or irregular heartbeat felt at rest
→
Dizziness, lightheadedness, or near-fainting
→
Pre-operative cardiac screening
→
Annual wellness evaluation with cardiac risk factors
→
Monitoring known heart conditions
|
📟 HOLTER MONITOR Continuous cardiac monitoring over 24-48 hoursA Holter monitor is a small wearable device that continuously records your heart’s electrical activity while you go about your normal daily life. You wear it for 24 to 48 hours, and during that time it captures every heartbeat — including any abnormal rhythms that may only occur intermittently and would be missed by a standard EKG. Commonly ordered when: →
Palpitations or racing heart that comes and goes
→
Unexplained fainting or near-fainting episodes
→
EKG was normal but symptoms continue
→
Monitoring effectiveness of heart rhythm medications
→
Evaluation after a stroke or TIA
→
Known atrial fibrillation — tracking frequency and duration
→
Sleep-related cardiac symptoms

A BRIEF HISTORY OF CARDIAC DIAGNOSTICS
From buckets of saltwater to a device the size of a smartphone
The science of reading the heart’s electrical signals has a remarkable history — spanning over 120 years and two Nobel-worthy inventions.
|
1901
The birth of the EKG Dutch physician Willem Einthoven invented the first practical electrocardiograph in 1901 using a device called a string galvanometer — a fine metal wire stretched between two powerful magnets. His original machine weighed 600 pounds and required five operators to run it. The first clinical EKGs were performed by connecting patients in a hospital ward to his lab via telephone cable — with their hands and one foot submerged in buckets of saline solution to complete the electrical circuit. Einthoven labeled the waves of the ECG tracing using the letters P, Q, R, S, and T — the same letters used on every EKG printed today, over 120 years later. 🏆 Einthoven received the Nobel Prize in Physiology or Medicine in 1924 “for the discovery of the mechanism of the electrocardiogram.” |
1949
The birth of the Holter monitor American biophysicist Norman “Jeff” Holter — a Montana physicist who had previously worked on the Manhattan Project and the first postwar atomic bomb tests at Bikini Atoll — turned his expertise in radio telemetry toward the heart. His first prototype in 1949 weighed 85 pounds — worn as a backpack, broadcasting cardiac data by radio. By 1968 it had been miniaturized to the size of a cigarette pack. Today’s Holter monitors are smaller than a smartphone. In a remarkable act of generosity, Holter donated the rights to his invention to medicine — ensuring it would remain accessible rather than locked behind a single commercial interest. 🏛️ The original Holter monitor is preserved at the Smithsonian Institution’s National Museum of American History. |
OUR PHILOSOPHY
Finding heart disease risk before it finds you
Heart disease is the leading cause of death in the United States — but it is also one of the most preventable. At PrimeCare of Novi, we take genuine pride in our ability to identify cardiac risk factors early, before symptoms develop and before a major event occurs. An EKG or Holter monitor is often a key part of that picture.
|
🔍
We look before symptoms appear Many serious heart conditions — including atrial fibrillation, left ventricular hypertrophy, and ischemic changes — produce no symptoms in the early stages. Routine EKGs at annual wellness visits allow us to detect these changes when they are most manageable. |
📊
We read the whole picture An EKG is one data point. We combine it with blood pressure trends, lipid panels, cardiometabolic labs, family history, and lifestyle factors to build a complete cardiovascular risk profile — and a plan to address it. |
🤝
We coordinate the next steps When results warrant cardiology referral, stress testing, or echocardiography, we coordinate that care directly and follow up to ensure nothing is missed between your primary care and specialist visits. |
“The best time to find a problem with your heart is before you have one.”
— The PrimeCare of Novi approach to cardiac prevention

PERSONALIZED CARE Want a physician who monitors your heart health proactively — year after year?In standard primary care, cardiac testing often happens reactively — after symptoms appear. In our Personalized Care program, we have the time and the relationship to take a different approach. Annual EKGs, comprehensive cardiometabolic panels, and trend analysis over years — not just a single data point at a single visit. Because our Personalized Care physicians manage a smaller patient panel, they genuinely know your history. When something changes on your EKG from one year to the next, your physician notices — and acts on it. ✓
Annual EKGs included as part of your wellness plan
✓
Comprehensive cardiometabolic lab panels
✓
Year-over-year trend tracking — not isolated snapshots
✓
Direct physician availability when results raise questions
✓
From the cost of a coffee a day
|
❤️ Why cardiac trends matter more than single results A normal EKG today does not mean the same thing as a normal EKG that looks slightly different from last year’s. Subtle EKG changes — a slight shift in axis, a small change in voltage, a new finding in the ST segment — can be clinically significant when compared against a baseline. Without a previous EKG for comparison, these changes are easy to miss. Personalized Care patients benefit from a longitudinal record that their physician knows intimately. That continuity is one of the most important — and least appreciated — aspects of preventive cardiology. |

COMMON QUESTIONS
Frequently asked questions about EKG and Holter monitors
Where can I get an EKG or Holter monitor in Novi, MI?
PrimeCare of Novi offers both in-office EKG and Holter monitor testing at 39555 W. Ten Mile Road, Suite 302, Novi, MI 48375. EKG results are reviewed same day. Holter monitors are worn for 24-48 hours and results are reviewed when the monitor is returned. Call (248) 426-7200 or request an appointment online.
What is the difference between an EKG and a Holter monitor?
An EKG captures about 10 seconds of your heart’s electrical activity while you are at rest in the office. A Holter monitor records continuously for 24-48 hours while you go about your normal daily life. The Holter monitor is used when symptoms are intermittent and unlikely to occur during a brief office visit — it captures abnormalities that a standard EKG would miss.
Does an EKG hurt? How long does it take?
No. An EKG is completely painless and non-invasive. Small sticky electrode patches are placed on your chest, arms, and legs. The machine records your heart’s electrical signals for about 10 seconds. The entire test takes less than 5 minutes from setup to completion. There is no radiation involved.
What can an EKG detect?
An EKG can detect a wide range of conditions including atrial fibrillation, ventricular arrhythmias, heart block, signs of a prior heart attack, left ventricular hypertrophy, bundle branch blocks, electrolyte abnormalities affecting the heart, and ischemic changes. It can also establish a baseline for future comparison — which is one reason we incorporate EKGs into annual wellness visits for patients with cardiac risk factors.
Can I shower or exercise while wearing a Holter monitor?
You should avoid getting the monitor wet — so showers and swimming are not recommended while wearing it. You can and should continue your normal daily activities including exercise. The goal is to capture what your heart does during your typical routine. Keep a diary of any symptoms you experience during the monitoring period — this helps your physician correlate your symptoms with the recorded data.
Is an EKG covered by insurance?
EKGs and Holter monitors are typically covered by most major insurance plans when ordered for a medical indication. We accept Aetna, Blue Cross Blue Shield, Blue Care Network, Priority Health, United Healthcare, Medicare, and most other major Michigan plans. Call our office at (248) 426-7200 to verify your coverage before your appointment.
Should I get a routine EKG even if I have no symptoms?
For patients with cardiac risk factors — including hypertension, diabetes, high cholesterol, obesity, smoking history, or a family history of heart disease — a baseline EKG is a reasonable part of preventive care. It establishes a reference point so that any future changes can be compared against it. Talk to your physician about whether a routine EKG makes sense for you at your next annual wellness visit.
