ADHD Assessment Questionnaires
These standardized rating scales are used by PrimeCare of Novi providers to evaluate attention and behavioral symptoms in children. Complete one or more scales below — scores are calculated automatically and can be printed and brought to your appointment or submitted to our office.
Conners Parent Rating Scale — Revised (Short)
Rate each item based on your child's behavior over the past month. Use the scale: 0 = Not at all, 1 = Just a little, 2 = Pretty much, 3 = Very much
Conners Teacher Rating Scale — Revised (Short)
Rate each item based on the child's behavior over the past month in your classroom. Use the scale: 0 = Not at all, 1 = Just a little, 2 = Pretty much, 3 = Very much
Vanderbilt ADHD Diagnostic Parent Rating Scale
Rate how often your child has shown each behavior over the past 6 months. Use: 0 = Never, 1 = Occasionally, 2 = Often, 3 = Very Often
Vanderbilt ADHD Diagnostic Teacher Rating Scale
Rate how often the child has shown each behavior over the past 6 months in school. Use: 0 = Never, 1 = Occasionally, 2 = Often, 3 = Very Often
Adult ADHD Self-Report Scale (ASRS v1.1)
Rate how often each situation has applied to you over the past 6 months. 0=Never 1=Rarely 2=Sometimes 3=Often 4=Very Often. Shaded columns on items 1-6 indicate a positive response for the primary screener.
