ADHD Assessment Questionnaires

These standardized rating scales are used by PrimeCare of Novi providers to evaluate attention and behavioral symptoms in children. Complete one or more scales below — scores are calculated automatically and can be printed and brought to your appointment or submitted to our office.

Important: These questionnaires are screening tools only and do not constitute a diagnosis. Results should be reviewed by a licensed healthcare provider. Please bring completed forms to your child's appointment at PrimeCare of Novi or call (248) 426-7200 to discuss results with your provider.

Conners Parent Rating Scale — Revised (Short)

Rate each item based on your child's behavior over the past month. Use the scale: 0 = Not at all, 1 = Just a little, 2 = Pretty much, 3 = Very much

Rate how often your child has shown each behavior over the past 6 months. Use: 0 = Never, 1 = Occasionally, 2 = Often, 3 = Very Often
